- Home Health Care is a health delivery
system wherein health care is rendered at the homes
of recovering, disabled, chronically or terminally
ill people in need of medical, nursing, social or
therapeutic treatment and/or assistance with the essential
activities of daily living.
- Grace Home Health Care is licensed to operate in
Los Angeles, Orange and San Bernardino County. At
present, we can ideally serve within a 15 to 20 mile
radius from our office.
- To be eligible for home health care services, the
following criteria must be met:
Patient must be homebound, i.e., a patient leaving
home would require considerable taxing effort
Services must be reasonable and necessary, i.e.,
the patient has an unstable and acute medical condition
that requires nursing intervention.
Services must require the skill level of the caregiver,
i.e., the patient needs skilled observation, monitoring,
assessments and training.
Services must be intermittent, i.e., it does not
include patient’s continuous care wherein
a ½ hour to 2 hours of nursing visit would
suffice.
Services must be provided under a plan of treatment,
i.e., the patient has a plan of care duly signed
by a physician.

It would really depend on each individual case.
Normally for a medicare beneficiary that is qualified,
medicare covers the bill. The same is true for Medi-Cal,
private insurances (depending on the plan) and HMOs.

- Yes we do. We have highly skilled nurses specializing
in pediatrics, IV infusion therapy, enterostomal care,
etc..
